OPEN TO ALL LEGAL RESIDENTS OF 48 CONTIGUOUS UNITED STATES AND
DISTRICT OF COLUMBIA, 21 YEARS AND OLDER AT TIME OF ENTRY
1. HOW TO ENTER: NO PURCHASE NECESSARY.
A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING.
Os-Cal® Bump Event Sweepstakes ("Sweepstakes") starts at 12:01.A.M. E.T. on 9/2/08 and ends at 11:59 P.M.,
E.T. on 12/19/08. Entries must be received no later than 11:59 P.M., E.T. on 12/19/08.
To enter, visit: www.oscal.com/bumpitup and provide all of the required information, including your name,
address, date of birth, home phone number, and email address.
Limit: One (1) entry per person per day regardless of email address.
All entries become property of GlaxoSmithKline® ("Sponsor") and will not be returned.
2. PRIZES & ODDS OF WINNING:
Grand Prize (1): One Grand Prize Winner will receive as the Grand Prize a trip ("Trip") for four (4) persons to
Hollywood, CA. The Trip consists of round trip coach airfare for four (4) persons from the major airport nearest
to Winner's residence to Hollywood, CA; 4 days/3 nights hotel accommodations (two [2] standard rooms/double
occupancy) at a hotel designated by Sponsor and a celebrity home tour. All Trip reservations and accommodations
will be as arranged by Sponsor. Grand Prize ARV: $7,152 depending on city of origin and dates of travel.
All expenses not specified above, including, but not limited to, meals, beverages, mini-bar, souvenirs,
gratuities, insurance, personal services, etc., are solely the Grand Prize Winner's responsibility.
A travel companion may not be a minor in his/her state of residence unless the Grand Prize Winner is his/her
parent/legal guardian/spouse. Grand Prize Winner and travel companions must possess required travel photo
identification documents and must travel on same itinerary. Trip must be completed by 9/1/09.
Sponsor has no liability for Grand Prize Winner's inability to utilize the Grand Prize.
Potential Grand Prize Winner must sign and return a notarized Affidavit of Eligibility/Compliance and
Publicity/Liability Release, where allowed by law, including providing his/her Social Security Number for
income tax purposes. S/he will also receive a Form 1099 as required by law. Failure to sign and return these
documents by the return date specified in the Prize notification letter will result in forfeiture of Grand
Prize and selection of an alternate potential Grand Prize Winner. Travel companions must also sign
Publicity/Liability Release, where allowed by law, prior to ticketing.
First Prize (1): One (1) First Prize Winner will receive a one (1) year's supply of Os-Cal
(4 bottles of Os-Cal 500 + D 210 ct.) for her/himself and a one (1) year's supply of Os-Cal
(4 bottles of Os-Cal 500 + D 210 ct.) to give to a friend. (ARV: $63.96).
(NOTE: Regardless of prize won, a prize may not be redeemed for cash; nor is it transferable or substitutable
except by Sponsor with one of equal or greater value. All federal, state and local taxes are the sole
responsibility of the Winner. Odds of winning depend upon the total number of all eligible entries received.
Limit: one (1) prize per person or household address.
3. RANDOM DRAWING:
On or about 1/18/09, the names of the potential winners will be selected by an independent judging agency,
in a random drawing from among all eligible entries received. The potential winners will be notified by mail
and/or phone on or about 1/24/09. If a potential winner is unreachable or if the potential Grand Prize Winner
does not return the documents stated in Rule 2 above by date specified in the prize notification letter, the
potential Grand Prize Winner will forfeit the Grand Prize in its entirety and an alternate potential Grand
Prize Winner will be selected. The independent judging agency's decisions will be final in all matters
pertaining to this Sweepstakes.
4. ELIGIBILITY:
Sweepstakes open to all legal residents of 48 contiguous United States or District of Columbia, 21 years of
age or older (residents of Alaska and Hawaii are not eligible). Officers, directors and employees
(and members of their household or immediate family, e.g., spouse, parents, children, siblings) of Sponsor,
their respective parents, subsidiaries, distributors, affiliates, sales representatives, participating retailers;
advertising and promotion agencies; or any other individuals or entities involved in the development or
administration of, or supply to, this Sweepstakes (hereinafter collectively referred to as "Sponsor & Others")
are ineligible to enter or win. Void in Alaska, Hawaii, Puerto Rico, Guam, U.S. Virgin Islands, and all other U.S.
Territories and Outlying Areas and wherever else prohibited or restricted by law.
5. LIMITATIONS OF LIABILITY:
Sponsor & Others are not responsible for illegible, lost, late, incomplete, stolen, misdirected postage due,
or undeliverable email, or postal mail; or for any computer, telephone, satellite, cable, network,
electronic or Internet hardware or software malfunctions, failures, connections, or availability, or
garbled, corrupt or jumbled transmissions, service provider/Internet/web site/use net accessibility,
availability, or traffic congestion, or any technical error, or unauthorized human intervention, or the
incorrect or inaccurate capture of registration information, or the failure to capture, or loss of, any
such information however caused or for any tampering, hacking of the equipment or programming associated
with or utilized in the Sweepstakes and assume no responsibility for any error, omission, interruption,
deletion, defect, delay in operation or transmission, communications line failure, technical error, theft
or destruction or unauthorized access to the Web site or any web site(s). Sponsor & Others are not responsible
for errors in the administration or fulfillment of this Sweepstakes, including without limitation; mechanical,
human, printing, distribution or production errors, and may modify or terminate this Sweepstakes based upon
such error at its sole discretion, without liability, and select potential winners from among all eligible
entries received prior to such modification or termination. Notice of such action by Sponsor will be posted
at the Sweepstakes web site. Sponsor and Others are not responsible for injury or damage to participants'
or to any other person's computer related to or resulting from participating in this Sweepstakes.
Sponsor reserves the right, to disqualify any individual found, in its sole opinion, to be tampering
with the operation of the Sweepstakes; to be acting in violation of these Official Rules; or to be
acting with the intent to disrupt the normal operation of this Sweepstakes. Any use of robotic, automatic,
macro, programmed, third party or like methods to participate in the Sweepstakes will void any attempted
participation effected by such methods and the disqualification of the individual utilizing the same.
CAUTION: ANY ATTEMPT TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES
IS A VIOLATION OF CRIMINAL AND CIVIL LAWS. SHOULD SUCH AN ATTEMPT BE MADE, THE SPONSOR RESERVES THE RIGHT TO
SEEK DAMAGES OR OTHER REMEDIES FROM ANY SUCH PERSON (S) RESPONSIBLE FOR THE ATTEMPT TO THE FULLEST EXTENT
PERMITTED BY LAW. IN NO EVENT WILL SPONSOR & OTHERS BE RESPONSIBLE OR LIABLE FOR ANY DAMAGES OR LOSSES OF ANY KIND,
INCLUDING DIRECT, INDIRECT, INCIDENTAL, CONSEQUENTIAL OR PUNITIVE DAMAGES ARISING OUT OF YOUR PARTICIPATION IN
THIS SWEEPSTAKES OR ACCEPTANCE OF ANY PRIZE. WITHOUT LIMITING THE FOREGOING, EVERYTHING ON THE WEBSITE IS
PROVIDED "AS IS" WITHOUT WARRANTY OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, IMPLIED
WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE OR NON-INFRINGEMENT. SOME JURISDICTIONS MAY NOT

6. PRIVACY POLICY:
Any personally identifiable information collected during an entrant's participation in this Sweepstakes will be
collected by Sponsor and its fulfillment agency and used by Sponsor, its fulfillment agency, and their
respective affiliates, agents and marketers for purposes of the proper administration and fulfillment of the
Sweepstakes as described in these Official Rules and in accordance with and Sponsor's Privacy Policy as stated at
http://consumer.gsk-chus.com/legal/privacy.aspx.
This Sweepstakes may not be published elsewhere without written permission from Sponsor.
7. PUBLICITY RIGHTS:
By accepting a prize, a winner agrees to allow Sponsor and/or
Sponsor's designee the perpetual right to use his/her name, photos and/or likeness, and statements for
promotion, trade, commercial, advertising and publicity purposes, at any time or times, in all media
now known or hereafter discovered, world wide, including but not limited to on the World Wide Web and
Internet, without notice, review or approval and without additional compensation except where prohibited by law.
8. GENERAL:
By entering this Sweepstakes, participants agree to abide and be bound
by these Official Rules. All Federal, state and local laws and regulations apply.
Sponsor & Others shall not be responsible for any damages, losses or injuries, whether property or personal,
including death, whether direct or indirect, arising out of or due to or in connection with acceptance,
use/misuse, or enjoyment of a prize or participation in this Sweepstakes or a Sweepstakes
related activity whether or not hosted by Sponsor, and they will be held harmless against any such
claims of liability. By entering this Sweepstakes, entrants agree that each entry becomes the
property of Sponsor and that such entry may be used by the Sponsor and its respective designees
and agents for any proper purpose. In the event of a dispute as to the identity of a winner
based on an e-mail address, the winning entry will be declared made by the authorized account
holder of the e-mail address submitted at time of entry. "Authorized account holder" is defined
as the natural person who is assigned to an e-mail address by an Internet access provider,
on-line service provider or other organization (e.g., business, educational institution, etc.)
that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address.
The interpretation of these Official Rules shall be governed by the laws of the Commonwealth of Pennsylvania.
Any and all disputes will be resolved individually, without resort to class action, in the appropriate courts
in the Commonwealth of Pennsylvania exclusively, and subject to the laws of the Commonwealth of Pennsylvania.
Should there be a conflict between the laws of the Commonwealth of Pennsylvania and any other laws,
the conflict will be resolved in favor of the laws of the Commonwealth of Pennsylvania.
Void in all jurisdictions outside of those listed above, and where prohibited by law.
9. WINNER LIST:
To obtain the names of the Grand Prize Winner, send a self-addressed stamped envelope, by 1/31/09,
